I think the definitive difference is in its photograph.
Its album cover is consists of four close-up shots, in which John, Paul, George and Ringo are "separated" from each other. On the other hand, their former album covers always features "Group Photos", in which four members are pictured together.
At that time, friendship between John and Paul had come as far as it was difficult to repair. The artwork is ensign of their serious conflicts and trouble. They were trying really hard to solve their problem and bring the project together again.
